INFORMATION services , *DIGITAL transformation , *SOCIAL problems , *INTERNET of things , *SUSTAINABLE development - Abstract
With the start of the digital transformation process, conventional information services’ current structure and presentation formats have been moved to a different dimension. The diversity of information carriers and the easy access of people to such devices have led to change and differentiation in information services, forcing service providers to produce innovative solutions. This situation has caused a necessary change in information services in super-intelligent societies that are tried to be created with human-robot cooperation. Society 5.0 aims to increase society’s welfare level and produce technology-based solutions to social problems by making use of the opportunities offered by technology. Society 5.0 is an equivalent word that corresponds to the expression super-smart society and is used with this meaning in the scope of the study. There aren’t enough studies in our country on Society 5.0, the foundations laid as a project in Japan in 2015. The subject is unknown, its importance cannot be understood, and the information services provided can experience problems adapting to the new generation of services. The study is aimed to follow the technological developments with the global agenda and to raise awareness for the renewal of the services offered in the information centers with these developments. The study was prepared based on the current literature, including the Society 5.0 project, the effects of the digital transformation process, and information services in super-smart societies. Introduction and Aim Antibiotics have an important role in the treatment of cystic fibrosis (CF) lung disease. Antibiotic resistance increases in patients with CF.The aim of this study was to determine whether there is a change in antibiotic resistance of respiratory cultures and susceptibility in respiratory tract specimens of patients with CF and to evaluate the relationship between the antibiotic resistance and patients' clinical status and pulmonary function tests. Materials and MethodsIn our study, patients followed with the diagnosis of CF at pediatric pulmonology clinic of Kocaeli University and the respiratory cultures which significant microorganisms were isolated from respiratory tract of CF patients from August 2010 through December 2016 were included and the clinical and laboratory findings of the patients were analyzed retrospectively. The respiratory culture results and the antibiotic susceptibility data were collected from the hospital's automation system. Results In this study, 333 respiratory cultures taken from 39 patients who were followed with the diagnosis of CF were analyzed. MSSA was isolated from 122 cultures (%31), P. aeruginosa from 97 cultures (%24,7) and MRSA from 22 cultures (%5,6) of 333 cultures. While MSSA and MRSA incidence increased in years, P. aeruginosa and other microorganisms' incidence didn't increased in years. MDR-MSSA and MDR-P. aeruginosa incidence was the same, MRSA incidence increased in years. There was a significant relationship between antibiotic resistance development and acute pulmonary exacerbation, pulmonary examination findings, percentile weight loss, percentile height loss. When antibiotic resistance was compared with PFT results, it was seen that as the antibiotic resistance increased, FEV1, FVC, FEV1 / FVC, PEF, FEF25-75 were significantly decreased. The use of azithromycin or inhaled antibiotics (in last 6 months, last 1 year, at any time) and the use of oral antibiotics in the three months before the culture were not significantly associated with the development of antibiotic resistance.ConclusionsIn conclusion, antibiotic resistance, especially multi-drug resistance (MDR), is a major problem in CF. Pulmonary disease remains the leading cause of morbidity and mortality in patients with CF. Therefore respiratory tract cultures should be taken at 3-month intervals and at the time of acute pulmonary exacerbation in patients with CF and the rational antibiotic treatment (correct antibiotic, correct pathway, correct and effective dose, correct duration) should be given according to the culture result. 61
